CRPS Treatment: pain in right hand for 18 years

Symptoms:Pain is limited to right hand and wrist and generally it is ranked at 8-9 on 10. The patient always wears a glove to protect from temperature, wind, water, human touch. The right hand is very weak, and the patient learned to do things with her left one.

Why the Patient decided to undergo the CRPS Treatment in Italy

Complex regional pain syndrome (CRPS) is a severely painful and disabling disease for which a multitude of therapeutic interventions have been proposed. In the past two decades, only Bisphosphonates gained credibility. In recent years, more convincing evidence has become available on the use of Parenteral Neridronate. Since 2014, this drug is registered and marketed in Italy for the treatment of CRPS. To date, the only therapeutic schedule that is recognized to be able to confer benefit is the intravenous (IV) administration of 100 mg given 4 times.

Daily Report

DAY 1: Arrival at the Venice airport, greeted by her Patient Manager, followed by check-in at her Hotel in Verona. Briefing focused on the next days.

DAY 2: Rest day in the hotel, in contact with her Patient Manager.

DAY 3: Rest day in the hotel, in contact with her Patient Manager. The patient feels very tired and has jetlag issues.

DAY 4: Morning pick-up by her Patient Manager, 1st visit with the Rheumatologist Professor, 1st Rehabilitation session, and 1st infusions. The patient is getting used to different time zone and she feels better.

DAY 5: Morning pick-up by her Patient Manager, 2nd infusions. The patient has a light fever, she takes some Tylenol to manage it.

DAY 6: Morning pick-up by her Patient Manager, 2nd Rehabilitation session and 3rd infusions. The patient still has some fever, she takes a long sleep in the afternoon which helps her.

DAY 7: Morning pick-up by her Patient Manager, 2nd visit with Rheumatologist Professor, 4th infusions.

DAY 8: Rest day. In the afternoon the patient has a long walk in Verona.

DAY 9: Pick-up by her Patient Manager and day trip to Venice. The patient feels well, and she enjoys it.

DAY 10: Pick up from her Patient Manager and visit of Vicenza center.

Day 11: Early pick-up from her Patient Manager and drop off Venice airport to fly back home.

Follow up

FEBRUARY 2023 (3 months after Neridronate treatment): the patient says that the pain is still there, even though it has decreased a bit, but she now wears her glove only when she goes outside her home.

APRIL 2023 (5 months after Neridronate treatment): the patient says that her pain level is down to 4/5. This is helping a lot in doing more activities and leaving home more often, her mood is much better.

JULY 2023: The pain level settled on a 4 and the patient refers that her hand is stronger than before. She can write again with her right hand after years.

JANUARY 2024: (14 months after Neridronate treatment). The patient says that her pain levels are still around 4, which she can manage compared to her state prior to the treatment. Her mood is still positive, she keeps on doing regular exercise.

Conclusion

Neridronate treatment is much more effective early after the CRPS onset, sometimes even leading to a full remission. After many years, as in this case after 18 years, some improvements are to be expected anyway, and can bring strength, peace of mind, and a sense of normality to patients who have been suffering for this long.
